Does it have nine pins, all in line along ONE EDGE ONLY of the black plastic? If so, this is a house number from the Cybernet factory for Toshiba type TA7310. Should still be available from folks who specialize in stuff for CB radios.
If the physical appearance is different from the one above, then that's not it.

Oooops, should have done a Google search before putting my foot in my mouth. It's a real, live PLL chip, and NOT the TA7310. Try Ken's Electronics. He had it listed, might still have some.
